The Bright Vision Approach

Personal Care Assistance (PCA) and the new Community First Services and Supports (CFSS) are Minnesota programs designed to help people with disabilities and older adults live independently in the community. At Bright Vision, we provide heart-centered support for activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Whether you need help with dressing and grooming or support with meal planning and household tasks, our dedicated team is here to ensure your needs are met with respect and kindness. As Minnesota transitions from PCA to CFSS, we are committed to guiding you through these changes so your support remains seamless and focused on your goals.

Every plan for this service is personally reviewed and guided by a Registered Nurse.

Who Qualifies for
This Support?

In Minnesota, eligibility is often determined through a combination of your health needs and program-specific criteria.

  • Individuals of any age with a disability.
  • Older adults needing help with daily tasks to remain at home.
  • Must be enrolled in Medical Assistance (MA) or a qualifying waiver.
  • Eligibility is determined by a MnCHOICES assessment through your county or tribe.
  • Must require help with at least one Activity of Daily Living (ADL) or exhibit a need due to behaviors.

To provide the safest care, it’s important to understand the professional boundaries of this service:

Cannot provide 24-hour-a-day home care under these programs.
Does not include specialized medical tasks that require a licensed nurse.
Services must be provided in the person's home or community, not a hospital or nursing home.
Authorization is based on your specific assessment and budget.
