Individual Community Living Supports (ICLS) is a bundled service designed specifically for older adults in Minnesota who are enrolled in the Elderly Waiver (EW) or Alternative Care (AC) programs. ICLS is about more than just 'doing' for you; it's about supporting your ability to manage your own home and stay connected to your community. This service includes six key components: active cognitive support, adaptive support, help with daily living activities, household management, health and wellness monitoring, and community engagement. At Bright Vision, we focus on what you can do, and provide the gentle reminders, cueing, and intermittent help you need to remain the leader of your own life in the home you love.

Every plan for this service is personally reviewed and guided by a Registered Nurse.

Eligibility

Who Qualifies for
This Support?

In Minnesota, eligibility is often determined through a combination of your health needs and program-specific criteria.

  • Seniors age 65 or older.
  • Must be enrolled in the Elderly Waiver (EW) or Alternative Care (AC) program.
  • Must require support in at least two of the six ICLS service components.
  • Intended for those who need reminders, cueing, or intermittent physical help rather than constant supervision.

To provide the safest care, it’s important to understand the professional boundaries of this service:

Not designed to meet a constant need for physical assistance (other than bathing).
Does not include 24-hour supervision or medical nursing care.
Cannot be used if you are receiving other similar waiver services like Individualized Home Supports.
Authorized only within your specific EW/AC service budget.
