Individualized Home Supports (IHS) with Family Training is a comprehensive service that recognizes the importance of the family unit. At Bright Vision, we don't just provide training to the individual; we extend that instruction to the family members and natural supports who live with them. This service is designed to improve the household's ability to support the person's goals, master daily routines, and manage health needs together. By building skills across the entire family, we help create a more stable, confident, and harmonious environment for everyone.

Every plan for this service is personally reviewed and guided by a Registered Nurse.

Eligibility

Who Qualifies for
This Support?

In Minnesota, eligibility is often determined through a combination of your health needs and program-specific criteria.

  • Individuals enrolled in a Minnesota disability waiver (CADI, BI, DD, CAC).
  • Must live with at least one family member or primary natural support person.
  • Must have a demonstrated need for training that includes family involvement.
  • Eligibility determined by your county or tribal case manager.

Limitations & Boundaries

To provide the safest care, it’s important to understand the professional boundaries of this service:

Family members cannot be paid to receive the training.
The training must be directly related to the person's authorized goals.
Cannot be provided if the person lives in a licensed residential setting.
Must be delivered by a 245D licensed provider like Bright Vision.
