An assessment identifies needs but does not guarantee funding (which comes from counties/insurers).
Care plans must be updated at least every 60-90 days depending on the program.
Does not replace the official MnCHOICES assessment performed by the county.
The plan is a guide and can be adjusted as often as needed with your input.
