What Does
"Person-Centered" Actually Mean?
At Bright Vision, "person-centered" isn't just a technical term. It means we build every service around the life you want to lead, focusing on four key pillars:
Personal Choice
You decide who supports you, when they visit, and how tasks are done.
Dignity & Respect
Every interaction is rooted in honoring your history, your home, and your privacy.
Community Focus
Your plan focuses on keeping you connected to the people and places you love.
Future Goals
We don't just maintain; we help you look forward to what you want to achieve.
Minnesota Compliant
Our care planning process meets or exceeds all 245D and MDH regulatory requirements for safety and transparency.

Ongoing Monitoring

Working With Your
Health Team
We don't work in isolation. Our care planning process involves constant communication with the professionals who already know your health history:
County Case Managers
We ensure your support plan aligns perfectly with your waiver (EW, AC, CADI) requirements.
Primary Care Physicians
Our RNs coordinate with your doctors to ensure medication and health goals are consistent.
Family & Loved Ones
We keep your family informed (with your consent) to ensure a complete circle of support.

What’s Inside Your
Support Plan
Your plan is a living document that guides our team in providing the exact support you need. It includes:
Navigating Your
Waiver Benefits
Most of our clients utilize Minnesota's Elderly Waiver (EW), Alternative Care (AC), or disability waivers. Our care planning process is designed to maximize your benefits while ensuring full compliance with program rules.
